DENVER – Not only were Colorado homes damaged by the recent severe storms, flooding, landslides or mudslides, but many survivors also lost valuable personal documents.  The documents include everything from Social Security cards to driver licenses to credit cards. 

The following is a partial list of ways to get duplicates of destroyed or missing documents:

Birth and Death Certificates – Birth and death certificates can be replaced by visiting your county vital records office or on line http://go.usa.gov/DFbw

Marriage Certificates – The online link for replacement of marriage certificates is http://go.usa.gov/DFbw

Marriage Dissolutions (divorces) – The online link for divorce decree replacements is http://go.usa.gov/DFbw

Adoption Decrees – The Colorado District Courts link for adoption records – if the adoption was finalized in Colorado – is http://go.usa.gov/DFbw

Immigration Documents – Contact your county office or the site below for citizenship, immigration, permanent resident card (green card), employment authorization, re-entry permit and more. uscis.gov

Driver Licenses – Visit any Colorado driver license office with acceptable identification and proof of address. Fee required.

Military Records – Request Standard Form 180 (SF-180) from any office of the Veterans Administration, American Legion, VFW or Red Cross, or download from http://tinyurl.com/lnu2pmt

Mortgage Papers – Contact your lending institution

Property Deeds – Contact the recorder’s office in the county where the property is located

Insurance Policies – Contact the insurance company for replacement papers

Social Security Card – Go to a Social Security Administration office. You also can request a copy of your Social Security statement online www.ssa.gov

Replacing Your Important Papers

NEW YORK – The Federal Emergency Management Agency understands that some survivors may need additional temporary rental assistance to help them achieve their permanent housing goals.

Most eligible applicants initially received four months of rental assistance and may be eligible for additional help, if their damaged residence is still not livable.

Applicants with an ongoing need for temporary rental assistance who have signed a valid lease agreement should:

  • Complete the “Declaration for Continuing Need for Rental Assistance” they received in the mail with all the applicable information and mail it back to FEMA at the address printed on the form.
  • Applicants will need to provide information of both pre-disaster and current expenses that are applicable to their households, such as pay stubs to verify income.
  • They also must provide a copy of their lease and the name and phone number of their landlord
  • Applicants who don’t have the form should contact FEMA’s helpline at 800-621-3362 (7-1-1 Relay or Video Relay Services are available) or (TTY) 800-462-7585.

The review process allows FEMA to be a good steward of taxpayer dollars by requiring applicants to demonstrate they have continuing housing needs.

FEMA recognizes this is a difficult time for Hurricane Sandy survivors and that some families need additional temporary assistance to assist them in their recovery. Therefore, families are encouraged to return to their repaired homes or to affordable housing as quickly as possible.

For continued rental assistance, survivors must re-certify

TRENTON, N.J. — Some households with property damage received a letter from FEMA that said “insufficient damage.” The FEMA housing inspector reported that the essential living areas in the home were safe, sanitary and functional. People who disagree can ask for a re-inspection.

FEMA can only provide rent or repair money when there is damage to the living room, dining room, kitchen, bathroom and bedrooms used by occupants of the home.

FEMA will reconsider a request for housing assistance, and there are other options:

  • Request a re-inspection. Call the FEMA helpline by phone or 711/VRS at 800-621-FEMA (3362), TTY 800-462-7585.
  • Complete and return your SBA low-interest disaster loan application if you received one. The U.S. Small Business Administration provides disaster loans for individuals. Loan funds may be available for damage to all living areas.
  • Get volunteer help. Dozens of private nonprofit groups are helping in New Jersey. Some  groups provide volunteers to help out with minor repairs to homes. For more information call 2-1-1 or visit NJ211.org online.
  • File an appeal. Any decision by FEMA can be appealed. Filing an appeal relating to habitability requires verifiable documentation to show the disaster caused damage that made the housing uninhabitable. Appeal letters must be postmarked within 60 days of the date of the decision letter.

For information on filing an appeal, survivors should refer to the “Help After Disaster” guide, which is mailed to everyone who registers with FEMA. The guide also is available online at www.fema.gov/help-after-disaster.

All applicants will receive a letter from FEMA. If you do not understand it, visit a disaster recovery center or call the FEMA helpline.
